We provide care from infants, children, and adolescents through the age of 18.

Patient portal

We have recently switched our patients over to their own patient portal!

This conveniently allows our patients to update their history, view their upcoming appointments, and complete and pre appointment forms!

This takes about 10 - 20 minutes to complete, if your child has an upcoming yearly physical and you have NOT set up a patient portal, we ask you come 20 minutes early to get them set up!

Primary Care Provider

Dr. Djafari received his Doctor of Medicine degree from the University of Tübingen, Germany. He completed his Residency at SUNY Health Science Center, Syracuse. He is Board Certified in Pediatrics in the U.S. and licensed to practice in the U.K. He is a fellow of the American Academy of Pediatricians and a Member of the American Society for Clinical Hypnosis. Dr. Djafari is fluent in English, Spanish, Italian, German, and Farci (Persian). He has two sons, Alex and Nich. In his leisure time he enjoys downhill skiing, tennis and classic movies.

Erin received her Certified Pediatric Nurse Practitioner degree, and Certification in Lactation Consulting at SUNY Upstate Medical University College of Nursing. She has experience in Pediatrics, Neonatal Intensive Care, Emergency Room, and ENT/Allergy. She is married to Adam and mother to Evangeline and William. In her free time, Erin enjoys hiking, camping, kayaking, gardening and baking.
